By becoming a member of Team Central Park, you commit to donate or raise funds of at least $1,000 for the benefit of the Central Park Conservancy—a designated Charity Partner of the 2008 NYC Half-Marathon Presented by NIKE. Your credit card information is required as a guarantee against this pledge. If you have not submitted at least one-half of the minimum pledge of $500 by the close of business on June 13, 2008, your credit card will be charged for the amount of $500, less any payments received from your donors by that date. If an initial credit card charge plus any payments from your donors do not total the full minimum pledge of $1,000, your credit card will be charged on July 18, 2008 for the amount necessary to fulfill that pledge. You understand that you are required to meet this final commitment date in order to maintain your guaranteed entry into the Marathon. The amount ultimately charged to your credit card will be considered a charitable donation and we will send you confirmation of this donation for tax purposes.
Contributions made by check should clearly identify that they are supporting your pledge by noting your name and the event in the memo field. Checks should be made payable to the Central Park Conservancy.
If you decide to leave Team Central Park prior to the final pledge commitment date, you agree to be charged the following amounts (if not already contributed) based on your leaving the team by the following dates: on or before May 16, 2008, one-quarter of the Pledge ($250); from May 17 through June 13, 2008, one-half of the Pledge ($500); from June 14 through July 3, 2008, three-quarters of the Pledge ($750); and after July 4, 2008, the full Pledge ($1,000).

WAIVER
You know that participating in the 2008 NYC Half-Marathon Presented by NIKE as a member of Team Central Park including pre-event training and related team activities (collectively, "the Event"), is a potentially hazardous activity. You agree not to participate unless you are medically able and properly trained. You agree to abide by any decision of an Event official relative to your ability to safely complete the Event. You are voluntarily entering and assume all risks associated with participating in the Event, including, but not limited to, falls, spills, contact with vehicles, other participants, spectators or others, the effect of the weather, including high heat, extreme cold and/or humidity, traffic and the conditions of the road, all such risks being known and appreciated by you. You grant to the Medical Director of the Event and his designee access to your medical records and physicians, as well as other information, relating to medical care that may be administered to you as a result of your participation in the Event.
Having read this Waiver and knowing these facts, and in consideration of your acceptance of this application, you, for yourself, and anyone entitled to act on your behalf, waive and release the Central Park Conservancy, Inc., the City of New York, the New York City Department of Parks & Recreation, and each of the respective representatives and successors, officers, directors, members, agents, and employees of the foregoing, from all present and future claims and liabilities of any kind, known or unknown, arising out of your participation as a member of Team Central Park even though that liability may arise out of ordinary negligence or fault on the part of the persons named in this Waiver. You grant permission to the foregoing persons and entities to use, or authorize others to use, any photographs, motion pictures, recordings, or any other record of your participation in the Event and as a member of the Team for any legitimate purpose without remuneration.
